A Vendor
Information Form must be
completed in full and submitted to the
Procurement Office to become a participating
vendor on our Bidder’s List.
Please send the
Vendor Information Form and any sales materials
such as catalogs to:

Once
the Vendor Information Form is approved, you
will be added to the Bidder’s List and will be
contacted when a quote is necessary.
Please note that if you fail to respond
to a quote request or decline to submit a quote
on three separate occasions, your name will be
deleted from the Bidder’s List.
