Faculty Clearance Form
Instructors should contact the Library Director (Copyright Officer) for assistance in documenting efforts to comply with copyright laws. If there is any doubt about copyright use, the matter should be discussed with the Copyright Officer and a completed copyright clearance form should be placed on file in the library.
