Job Description:

Primary Purpose: To work closely in a collaborative team environment to implement the service delivery for each participant and their family, by coordinating all the services a participant may need, beginning with intake.

 

Primary Responsibilities:

  • Functions as a team member in interviews, development and implementation of the Plan of Service, ongoing reviews, case coordination, and management of daily service delivery
  • In conjunction with the team, conducts a Needs Assessment at intake and at end of 90 days
  • Prepares clients and assists in contacting needed community resources such as food stamps, applying for Medicaid, assisting with enrolling children in school/daycare programs
  • Develops and maintains a RESOURCE reference guide
  • Submits Request for Funds vouchers to be signed as need occurs for clients
  • Effectively supervises client volunteer work and documents hours worked
  • Participate in team individual meetings with director and staff
  • Participates in developing and following through with Individual Crisis Management Plans designed for clients
  • Maintains documentation per program standards and participates in staff training opportunities offered on campus.
  • Successfully completes required training and demonstrates skills in Fire Safety, First Aid and CPR training, C.A.R.E (Children and Residential Experiences) and T.C.I. Therapeutic Crisis Intervention training
  • Submits requests for training offered by conferences and other organizations relevant to the job description and/or program
  • Interact daily with client families taking note of any special need, behavior, or emotional upset to Case Management Coordinator that may need to be addressed in counseling
  • Encourage and demonstrate daily life skills with families; including parenting and job readiness skills
  • Updates daily logs and reports for needed supplies, repairs, or damages to facility, grounds, or furnishings
  • Assists the team in decorating the cottage for Christmas
  • Program-related transportation for clients that may occur when Transport Support staff not available
  • Manages cottage household, supporting and monitoring cottage rules, schedule and chores
  • In conjunction with the Support Specialist, assist clients with menus, recipes, and meal preparation
  • Follows approved guidelines for making cottage purchases
  • Achieve yearly "Gold Star" list
  • Other duties as assigned by the Supervisor
