We are seeking a highly motivated and innovative professional to serve as the Community
Coalition Coordinator. This role is designed for a leader who can bring vision, discipline, and
collaboration to the work of improving health and strengthening community systems in
McCormick County.

of healthier, stronger, more connected communities.
Professional Overview
The Coordinator will lead the development of the Healthy People, Healthy Carolinas (HPHC)
grant through the Moving Forward McCormick Coalition (MFMC). This position blends
strategic leadership with hands-on coordination to ensure that programs, partnerships, and
systems produce measurable community impact.
Position Summary:
The Community Coalition Coordinator will oversee the components of the Moving Forward
McCormick Coalition (MFMC) and is primarily responsible for:
● Developing and/or community organization relationships;
● Building operational readiness to implement the HPHC grant in conjunction with the
MFMC;
● Providing general oversight for the HPHC grant coalition activities and associated
projects;
● Helping to identify and recruit additional cross-sector stakeholders to participate in the
coalition work;
● Grant development, submission and management;
● Ensuring effective facilitation of all work group meetings and providing regular reports on
group progress against goals and indicators;
● Collaborating with local/regional coalitions and partnerships that focus on chronic
disease reduction;
● Working to develop systems and implement strategies that will drive transformative
change in the community;
Key Responsibilities
● Coordinate community health initiatives and support the implementation of evidence-based
interventions
● Facilitate monthly coalition meetings and maintain regular communication with partners
● Build relationships with key community stakeholders to advance shared goals
● Ensure timely reporting for funders and maintain program documentation
● Support coalition in strategic planning, program evaluation, and action planning
● Manage budget and assist with grant writing to secure additional resources
● Represent the coalition at local and state meetings and events
● Maintain coalition website and share program updates
● Complete other duties as assigned
