ADDITIONAL INFORMATION:

Education Requirements:

  • Minimum Associates Degree in a Health Care field; Bachelor's Degree preferred

Work Experience:

  • A minimum of 2-years of work experience preferably in a Health Care setting. Preferred knowledge of Managed Care Medicare Advantage programs.
Job Description:

Urgently hiring. This position is a temporary-to-possible-permanent role.

 

Duties and Responsibilities:

  • Develop and maintain current understanding of Medicare Risk Adjustment measures and quality incentive programs and requirements
  • Review paperwork from Managed Care Organizations (MCOs) and work with provider offices to get this paperwork completed
  • Works directly with contracted Medicare Advantage MCOs to manage their programs and maximize incentives offered by each company
  • Work directly with Chronic Care Management Program to ensure completion of process to optimize payment
  • Target specific patient populations to ensure all patients receive evidence based health maintenance and chronic disease services
  • Contact patients enrolled with MCOs but that have fallen out of care or those who have not yet been established with a provider at CHC
  • Ensure access to care to all CHC services as well as those in the community
  • Document reminders to providers in patients’ chart in the electronic health record system
  • Perform any other duties as assigned by your supervisor
