REQUIREMENTS:
Skills:
- Able to read, write and communicate effectively in writing
- A working knowledge of clinical terms
- Proficient in use of computer and keyboard
- Experience with Excel spreadsheets
- Able to establish and maintain effective working relationships
- Knowledge of HIPAA and ability to maintain confidentiality
- Effective critical thinking and problem solving skills necessary for coordinating care
- Effective communication skills necessary to clearly inform and educate patients

Physical Abilities:

ADDITIONAL INFORMATION:
Education Requirements:
- Minimum Associates Degree in a Health Care field; Bachelor's Degree preferred
Work Experience:
- A minimum of 2-years of work experience preferably in a Health Care setting. Preferred knowledge of Managed Care Medicare Advantage programs.
Urgently hiring. This position is a temporary-to-possible-permanent role.
Duties and Responsibilities:
- Develop and maintain current understanding of Medicare Risk Adjustment measures and quality incentive programs and requirements
- Review paperwork from Managed Care Organizations (MCOs) and work with provider offices to get this paperwork completed
- Works directly with contracted Medicare Advantage MCOs to manage their programs and maximize incentives offered by each company
- Work directly with Chronic Care Management Program to ensure completion of process to optimize payment
- Target specific patient populations to ensure all patients receive evidence based health maintenance and chronic disease services
- Contact patients enrolled with MCOs but that have fallen out of care or those who have not yet been established with a provider at CHC
- Ensure access to care to all CHC services as well as those in the community
- Document reminders to providers in patients’ chart in the electronic health record system
- Perform any other duties as assigned by your supervisor
