Job Description:
Social Services / Social Worker. Excellent customer service skills required. Works closely with residents, families, staff and medical professionals. Experience with and knowledge of MDS assessment and care plan process. Coordinate care plan meetings as needed. SSD will work closely w/ IDT in the admissions, case management and D/C planning process. Documentation skills a must.
Schedule:
Primarily day shift, flexible hours as needed for departmental / facility needs.
